  3. Tracking Risk and Progress If someone is marked as high-risk (e.g., underweight baby, complicated pregnancy, or missed vaccines), the system automatically flags it for urgent follow-up.

  4. Doorstep or Local Checkups Nurses either visit homes or conduct checkups at a nearby center. Any concerns are reported to doctors, who can advise remotely if needed.
